• New Evaluation (90 min) ______________________ $375
• Follow up Visit (30 min) _______________________ $175
• Psychotherapy (45-50 min.) ____________________ $300
• Late Cancellations or No Shows __________________$50

Lawrence M. Martin MD PC is a solo private practice of psychiatry. It has been determined that being a provider for multiple insurance companies at least triples the administrative work, substantially reduces payment, and adds significant constraints to providing optimum care to each patient. To this end, it has been determined that WE DO NOT TAKE INSURANCE ASSIGNMENT INCLUDING MEDICARE AND OTHER GOVERNMENT INSURANCE. IF YOU HAVE THIS TYPE OF INSURANCE YOU WILL NEED TO SIGN A WAIVER AGREEING NOT TO SEEK REIMBURSEMENT FOR SERVICES PROVIDED.  A waiver is included in the packet and will need to be signed prior to receiving services.


ALL PAYMENTS ARE DUE AT TIME OF SERVICE. While we do not contract with or submit to insurance we will provide you with a “Superbill” with all the necessary information for you to submit to your insurance company for reimbursement.

A FEE OF $50 WILL BE CHARGED FOR A NO SHOW OR LATE CANCELLATION OF ANY TYPE OF RETURN VISIT. If there is a pattern of late cancellations or no shows, you will be required to pay for your next appointment in full before an appointment will be made. We understand that there are occasional unavoidable situations that may lead to the No Show or Late Cancellation. However, consistent application of this policy is the only way to reinforce the importance of your care. It is our hope that you will come to value the care you receive enough to understand the need for this requirement. We do try to make courtesy reminders but it is your responsibility to keep track of and come to your appointments.

A $25 fee will be charged for all returned checks. The patient agrees to reimburse this office for any collection and /or legal charges incurred as a result of patient’s failure to honor his/her bill.
